Come to think of it this happens to me also (as you stated mostly around people i know), do you think it's the added stress that makes it worse? as you are around people you know, as when i'm around total strangers it never happens.

I definitely find myself being hyper aware of any slight error on my own part and am often over hard on myself.I also have a tendency to over analyse everything I do.

I do have some issues where I feel people are judging me though I find it's strangers who don't know me in my case.

It's when I have those days where everything seems to be going wrong that I start forgetting important things.
