Agreed. I was on the debating team at high school, most of the debates in the senior years were impromptu or limited-prep. I have exactly the same problem. Five years worth of lessons yielded little progress. I find that if the melodies have the same rhythm, or only break rhythm for little arpeggios that fit within a right hand crotchet or whatever, I can struggle through them. I could stand to be corrected on this. for example is driving a nail in to something fine or gross? or perhaps a combination? I think you're right; it's both.
